Contract for Board Certified Elder Law Attorney

As effective date agreement, contract (the “Contract”) entered undersigned parties, hereinafter referred “Client” “Attorney.”

1. Scope Representation
The Attorney agrees to provide legal services to the Client in the area of elder law, including but not limited to estate planning, long-term care planning, Medicaid and Medicare planning, guardianship and conservatorship, and other related matters.

The Attorney shall exercise due diligence and professional competence in representing the Client, in accordance with the standards of professional responsibility and ethics set forth by the American Bar Association and relevant state laws.
